CareMonitor - Patient - Provider Partnership Platform wins ADHA Award

One of our member practices, Mt Druitt Medical Centre is the first practice to start using CareMonitor for patients as part of their routine care.  CareMonitor is an app which allows patients to connect with their care team as well as allowing the care team to better communicate with each other and co-ordinate care.CareMonitor has been awarded the Australian Digital Health Agency 2020 Innovation award .
